Pride Health is hiring a
Registered Nurse – Hospice Case Manager for a 9-week contract assignment supporting a leading healthcare organization in
Columbia, SC. This role is ideal for an experienced Hospice RN who is passionate about providing compassionate patient care while collaborating closely with patients, families, and the interdisciplinary team.
Responsibilities Include:
• Provide skilled nursing care and manage the medical needs of assigned hospice patients, including routine patient visits and bedside documentation.
• Participate in agency meetings, patient care conferences, and IDG meetings by preparing and presenting patient updates.
• Build strong relationships with patients, families, and staff while providing education, support, and compassionate care.
• Adapt to changes in scheduling based on patient needs and staffing requirements.
Qualifications:
• Active Registered Nurse (RN) license in South Carolina.
• Hospice nursing experience required.
• HCHB (Homecare Homebase) experience preferred.

Schedule: Monday–Friday, 8:00 AM – 4:30 PM

Hours: 8-hour shifts / 40 hours per week

On-Call Requirement: Yes, as needed
•
Coverage Area: Up to 55 miles from office location, primarily serving Richland and Lexington Counties
•
Pay Range: $53.00–$57.00/hour
•
Mileage Reimbursement: Yes, mileage reimbursed between patient visits
