RN Hospice Case Manager – Travel Contract

Location: South Dennis, Massachusetts
Contract Duration: 13 Weeks
Schedule: Monday – Friday | Day Shift
Hours: 08:00 AM – 04:00 PM | 40 Hours/Week
Setting: Hospice Home Health / Home Care

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced and compassionate Registered Nurse (RN) Hospice Case Manager for a 13-week travel assignment supporting hospice patients across home and care facility settings throughout the South Dennis, Massachusetts.

The ideal candidate will have strong hospice case management experience, excellent clinical judgment, and a passion for providing compassionate end-of-life care to patients and families.

Key Responsibilities

  • Provide skilled nursing care and hospice support in home and facility settings
  • Manage hospice patient caseloads and coordinate interdisciplinary care plans
  • Assess patient conditions and respond appropriately to changes in health status
  • Develop, implement, and update individualized care plans
  • Deliver pain and symptom management focused on patient comfort and quality of life
  • Participate in interdisciplinary team meetings and case conferences
  • Communicate patient condition updates with physicians and healthcare teams
  • Complete admissions, assessments, and clinical documentation accurately and timely
  • Educate patients, caregivers, and families regarding hospice care and treatment plans
  • Ensure compliance with hospice regulations, standards, and policies
  • Supervise Hospice Aides as required
  • Travel to patient homes within the assigned coverage area

Required Qualifications

  • Active Massachusetts RN License
  • Minimum 2 years of Hospice RN Case Management experience
  • Previous Hospice Home Care experience required
  • Strong patient assessment and case management skills
  • Excellent communication and organizational abilities
  • Ability to work independently in a fast-paced environment
  • Strong critical thinking and compassionate patient care skills
  • Valid driver’s license and reliable transportation
  • Basic computer proficiency

Preferred Qualifications

  • BSN preferred
  • IV Certification preferred
  • Previous VNA or Home Hospice experience preferred
