Responsibilities:
Hospice RNat the DHHC focus on providing pain management and comfort for patients with terminal or chronic illness, as well as communicate and coordinate care needs with the interdisciplinary team, the patient and their families/caregivers.
The RN:
· Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.
- Coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate.
- Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals. Communicate significant changes to the patient's physician and DHHC colleagues.
- Document evaluations, patient response, and progress towards goals.
- Teach the patient and caregiver care techniques and end-of-life symptom management as appropriate.
Qualifications:
- Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
- Minimum of one year of experience required.
- Graduate of an accredited school of nursing.
- Medical/Surgical/Oncology nursing and/or hospice care experience preferred.
Required Licensure/Certifications:
- Driver’s license, minimum requirements on car insurance, and a clean driving record
