Job Description

Position Overview

Seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join our team serving the Greater Lake Charles area. This role is responsible for coordinating and managing patient care plans, ensuring exceptional end-of-life care, and providing support to patients and their families. The ideal candidate is patient-centered, adaptable, and committed to delivering high-quality hospice services.

Schedule & Work Details

  • Location: Greater Lake Charles coverage area
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hour Guarantee: 40 hours
  • Pay Rate: $51-$53 per hour
  • On-Call/Weekend Requirement: None
  • Mileage Reimbursement: Yes, mileage is reimbursed between patient visits

Key Responsibilities

  • Manage and coordinate care for an assigned caseload of hospice patients.
  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Monitor patient conditions and adjust care plans as needed to meet changing needs.
  • Collaborate with physicians, interdisciplinary team members, patients, and families to ensure quality care delivery.
  • Provide skilled nursing care, symptom management, and patient/family education.
  • Maintain accurate and timely clinical documentation.
  • Advocate for patients and families while supporting their physical, emotional, and psychosocial needs.
  • Ensure compliance with hospice regulations, policies, and standards of care.

Required Qualifications

  • Active Registered Nurse (RN) license.
  • Previous hospice nursing experience.
  • Strong assessment, clinical, and case management skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Excellent communication and organizational abilities.

Preferred Qualifications

  • Experience with Homecare Homebase (HCHB).
  • Strong patient-centered approach to care.
  • Positive, flexible, and team-oriented attitude.
  • Ability to adapt to changing patient needs and priorities while maintaining high-quality care standards.
