    The RN Hospitalist is a registered nurse who collaborates with hospitalists, unit staff, and the interdisciplinary team to coordinate and manage inpatient care from admission through discharge and handoff to outpatient providers. The role synthesizes assessment findings to support planning, implementation, and evaluation of care; promotes efficient use of resources; and ensures safe, timely transitions, including patient/family education, follow-up appointments, and linkage to post-acute services. The RN identifies patients with complex medical or social needs and helps develop strategies to reduce readmissions and improve outcomes. Success in this role requires strong clinical judgment, communication, and organization skills, a commitment to service/logical collaboration, and practice within the scope of licensure under nursing and hospitalist leadership.

    Responsibilities

    Patient Care:

    • Provides bedside marketing to educate patients & families on the Hospitalist Program; verifies primary care provider, identifies barriers to discharge, and sets expectations for the hospital stay
    • Actively participates in morning report/hand-off and may serve in the role of census coordinator
    • Collaborates with the care team to ensure labs, test results, consults, and orders are completed in a timely fashion
    • Initiates proactive planning and coordination of the patient's plan for the day and plan for the stay with the team
    • Serves as continuity for patients during physician hand-offs; collaborates with physicians and care coordinators regarding care delays
    • Obtains medical records and assists with medication reconciliation; sets up care team meetings with physicians and families as needed
    • Acts as a patient advocate, maintaining privacy and confidentiality

    Care Coordination & Transitions of Care:

    • Collaborates with Hospitalists and Care Coordination discharge planning to ensure appropriate and safe discharge plan and disposition for identified patient population
    • Resolves post-discharge issues and provides homecoming calls to follow-up with high-risk patients; triages patient-related phone calls to the Hospitalist office
    • Monitors labs and other pending items post discharge; forwards abnormal results to the appropriate provider for follow-up
    • Assists with discharge orders and clarifies discharge prescriptions; completes paperwork for patients and families
    • Coordinates outside hospital labs and records; ensures proper communication with the patient's PCP
    • Educates patients at the bedside and over the phone, recognizing when collaboration with a physician is needed; coordinates care until the patient has been seen by an established or new PCP
    • Communicates continuation of patient needs outside of the hospital when processes do not meet patient needs

    Interdisciplinary Care Coordination & Quality Improvement:

    • Acts as a liaison between the Hospitalist care team, care coordinators, and other members of the patient's health care team (e.g., unit nurses, PT, OT, outpatient providers)
    • Participates in interdisciplinary care conferences to review the plan of care for select high-risk patients
    • Serves as a conduit to the next level of care to facilitate a successful transition (referrals, medical imaging procedures, CMN, etc.)
    • Participates actively in quality improvement initiatives; reviews core measure indicators and supports achievement of team goals; improves the efficiency of the hospitalist care team
    • Participates in patient satisfaction related issues and educates/promotes the Hospitalist program among hospital staff

    Qualifications

    Education:

    • Graduate of accredited nursing program (Must obtain BSN within 5 years of hire)

    Experience:

    • two (2) years hospital experience with demonstrated communication skills, ability to work effectively in a team, and commitment to continuous quality improvement
    • three (3) years of nursing experience preferred
    • one (1) year of home care or outpatient clinic experience preferred
    • one (1) year of experience in utilization management, case management, or hospital discharge planning preferred

    Certificate/License:

    • Current licensure in good standing to practice as a Registered Nurse in Iowa
    • Must posesses and maintain current Basic Life Support (BLS) certification
