The RN is responsible for assessing patient needs, administering infusion treatments, monitoring patient responses, educating patients and caregivers, and communicating changes in patient condition to the care team.The ideal candidate will have strong vascular access skills and experience managing a variety of infusion therapies in the home setting.ResponsibilitiesProvide skilled nursing care and infusion therapy services in patients' homesImplement and manage individualized plans of care based on physician ordersPerform comprehensive nursing assessments and ongoing patient evaluationsMonitor patient response to treatments and promptly report significant changes in conditionAdminister prescribed infusion therapies safely and effectivelyPerform peripheral IV insertions and maintain vascular access devicesProvide care, maintenance, and infusion services through:PICC lines, Central Venous Lines (CVL), Midlines (ML), Peripheral IVs, Port-a-CathsEducate patients and family members regarding medications, infusion therapy, disease management, and treatment plansCommunicate effectively with physicians, pharmacists, patients, caregivers, and the Director of NursingMaintain accurate, detailed, and timely clinical documentationFollow all agency policies, infection control procedures, and nursing practice standardsTreat patients, families, and team members with professionalism, courtesy, and respectQualificationsCurrent, unrestricted Texas Registered Nurse (RN) licenseCurrent BLS/CPR Certification (in-person certification required)Negative TB skin test or chest X-ray within the past yearMinimum one (1) year of documented nursing experience in a home health agency, primary care clinic, infusion setting, or healthcare facility requiredStrong peripheral IV insertion skills requiredExperience providing infusion therapy and managing vascular access devices preferredCurrent and valid Texas Driver's LicenseCurrent automobile insuranceReliable transportationStrong assessment, communication, and documentation skillsPhysical RequirementsAbility to stand, walk, bend, squat, kneel, reach, twist, push, and pull throughout the workdayAbility to safely assist patients as needed during care visitsAbility to independently lift up to 50 poundsJoin Our TeamIf you're an experienced RN with strong IV and infusion skills who is passionate about delivering exceptional patient care, we'd love to hear from you.
