Coordinate medical, behavioral, and community-based services across the continuum of care. Collaborate with physicians, specialists, caregivers, and community organizations to ensure members receive appropriate care. Monitor member progress, evaluate changes in condition, and update care plans as needed. Conduct home and/or community visits (typically once per month or as required) to assess member needs. Review referrals and intake assessments to determine appropriate care management strategies. Educate members and caregivers on disease management, available benefits, healthcare resources, and community services. Coordinate service authorizations for long-term care services such as transportation, meals, housing, and activities of daily living. Maintain accurate documentation and ensure compliance with state, federal, and organizational guidelines. Identify care gaps, monitor quality outcomes, and recommend improvements to enhance member care. Serve as a liaison and advocate between members, families, healthcare providers, and community resources. Support onboarding and mentoring of new clinical staff as needed.Key Requirements and Technology Experience: Key skills: - ("RN", "Long term care", "LTC", "Home visits", "Case Management") Graduate of an accredited School of Nursing. Active Registered Nurse (RN) license in the state of practice or Compact State Licensure. Bachelor's degree with related clinical experience preferred. 4–6 years of healthcare or care management experience preferred (not required). Ability to travel locally for member visits.
