Position Overview
A healthcare organization in Northern California is seeking an experienced RN Manager to lead Care Management operations within an acute care environment. This leadership role is responsible for overseeing care coordination activities, supporting interdisciplinary collaboration, and ensuring patients receive appropriate services across the continuum of care.
The RN Manager will guide case management processes that promote effective transitions of care, resource utilization, patient advocacy, and improved health outcomes. The position works closely with patients, families, physicians, social workers, payers, community partners, and post-acute providers to ensure comprehensive and coordinated care delivery.

Key Responsibilities
  • Lead and support Care Management and care coordination initiatives.
  • Oversee patient assessment, care planning, implementation, monitoring, and evaluation activities.
  • Promote effective communication among interdisciplinary care teams.
  • Ensure appropriate utilization of healthcare resources and services.
  • Facilitate safe and efficient transitions of care across healthcare settings.
  • Address complex patient needs, including psychosocial, behavioral health, substance use, resource, and social determinant of health challenges.
  • Support quality improvement initiatives and operational performance goals.
  • Provide leadership, mentorship, and guidance to Care Management staff.
  • Collaborate with community and post-acute care partners to ensure continuity of care.
Required Qualifications
  • Bachelor's Degree in Nursing.
  • Active California Registered Nurse (RN) license.
  • Minimum 5 years of healthcare experience.
  • Minimum 3 years of Case Management experience within an acute care hospital setting.
  • Minimum 1 year of healthcare leadership or management experience.
Preferred Qualifications
  • Master's degree in Healthcare Administration, Nursing, or a related field.
  • Certified Case Manager (CCM) credential.
  • Accredited Case Manager (ACM) credential.
