Position Overview
An established acute care hospital is seeking an experienced RN Manager to lead its Case Management department. This leadership role is responsible for overseeing daily operations, clinical practice standards, staff development, quality initiatives, and care coordination programs that support patients across the continuum of care.
The department works collaboratively with patients, families, physicians, nurses, social workers, payers, community resources, and post-acute providers to ensure effective care transitions and appropriate utilization of healthcare resources.
Key Responsibilities
  • Direct the daily operations of the Case Management department.
  • Lead and support Care Managers and departmental support staff.
  • Promote effective care coordination and transitional care planning.
  • Ensure compliance with organizational policies, regulatory requirements, and quality standards.
  • Monitor department performance, productivity, and budget goals.
  • Analyze quality metrics and implement performance improvement initiatives.
  • Develop and implement department programs, strategies, and operational plans.
  • Foster a collaborative and highly engaged team environment.
  • Oversee recruitment, onboarding, training, professional development, and retention of staff.
  • Support patient advocacy, resource management, and coordination of services across the healthcare continuum.
  • Collaborate with interdisciplinary teams to address complex patient needs, including social determinants of health and discharge planning challenges.
Required Qualifications
  • Bachelor's Degree in Nursing.
  • Active California Registered Nurse (RN) License.
  • Minimum of 5 years of healthcare experience.
  • Minimum of 3 years of Case Management experience in an acute care/inpatient hospital setting.
  • Minimum of 1 year of healthcare leadership or management experience.
Preferred Qualifications
  • Master's Degree in Healthcare, Nursing, or a related field.
  • Certified Case Manager (CCM) credential.
  • Accredited Case Manager (ACM) credential.
