Qualifications
Required:
Current, unrestricted Registered Nurse (RN) license in New York State
Graduate of an accredited school of nursing
Valid New York State driver's license
Reliable transportation and automobile insurance
Experience as a Registered Nurse
Excellent leadership, communication, and organizational skills
Preferred:
Bachelor of Science in Nursing (BSN)
Previous supervisory or management experience
Home Care (LHCSA or CHHA) experience
Experience with staff development, quality improvement, and regulatory compliance
Key Responsibilities
Provide clinical oversight and ensure the delivery of exceptional patient care.
Perform nursing assessments and develop individualized aide care plans based on physician orders and patient needs.
Conduct supervisory visits and update care plans as needed.
Supervise and mentor Home Health Aides (HHAs), Personal Care Aides (PCAs), and branch support staff.
Collaborate with Scheduling Coordinators to maximize staffing efficiency and patient coverage.
Participate in recruiting, interviewing, hiring, and onboarding qualified staff.
Provide coaching, performance evaluations, and ongoing staff education.
Promote infection control practices and ensure compliance with agency policies.
Monitor quality improvement initiatives and regulatory compliance.
Participate in the nursing on-call rotation.
Communicate changes in patient condition with physicians and interdisciplinary team members.
Conduct home safety assessments and provide patient and caregiver education.
Build relationships with physicians, hospitals, and community referral sources to grow services.
Represent VNHC at networking and community events.
Assist with PCA/HHA training programs as needed.
Maintain current knowledge through continuing education and professional development.

Position Summary
