Position Summary
The MDS Coordinator (RN) is responsible for coordinating and completing comprehensive resident assessments in accordance with federal and state regulations, facility policies, and quality standards. This role plays a key part in ensuring accurate clinical documentation, care planning, reimbursement optimization, and regulatory compliance while supporting the delivery of exceptional resident care.
Key Responsibilities
- Conduct and coordinate the completion of resident assessments (MDS) in accordance with federal and state requirements.
- Ensure the timely and accurate completion of MDS, Care Area Assessments (CAAs), and care plans.
- Collaborate with interdisciplinary team members to develop and maintain individualized resident care plans.
- Monitor assessment schedules and ensure compliance with Medicare, Medicaid, and regulatory guidelines.
- Review clinical documentation to support accurate coding and reimbursement.
- Assist with surveys, audits, and quality improvement initiatives.
- Serve as a clinical resource for nursing staff regarding assessment and documentation requirements.
- Maintain current knowledge of MDS regulations and reimbursement processes.
Qualifications:
Required
- Nursing degree from an accredited college or university.
- Current, active, and unencumbered Registered Nurse (RN) license in the state of practice.
- Minimum of one (1) year of experience in a hospital, long-term care facility, or related healthcare setting.
- Experience completing and coordinating MDS assessments in a long-term care or skilled nursing facility.
- Strong organizational, analytical, and communication skills.
- Knowledge of federal and state regulations related to resident assessments and care planning.
Preferred
- RAC-CT certification or advanced MDS training.
- Experience with Medicare, Medicaid, and skilled nursing reimbursement processes.
- Previous experience in long-term care clinical leadership or care coordination roles.
