RN/MSW Case Manager

Job Overview:

Can you provide Case management Services in accordance with acceptable social service practices and hospital policies and procedures? Then we want you! This position needs someone who can assist patients and their families with personal and environmental difficulties with predispose illness or interfere with obtaining maximum benefits from medical care by performing the following duties:

• Works in close collaboration with members of the healthcare team to provide services and optimize patient functioning in the most efficient and effective manner. Performs comprehensive evaluation including social, emotional, cultural, mental status, environments, and financial circumstances in conjunction with interdisciplinary assessment, and recommends a plan of intervention based on mutually established patient goals
• Must be able to demonstrate knowledge of growth and development to coordinate patient care appropriate to the age of patients served on his/her assigned clinical area. Employee must be able to successfully complete job specific orientation Case Management specific competencies and meet/exceed standards as identified in the criteria based on performance appraisal.

Position Requirements:

MSW Case Manager 

• Graduate of a program accredited by the Council on Social Work Education with a Masters of Social Work (MSW).
• Experience in health care, crisis intervention, counseling and/or social agency preferred. This can include internships or other employment. 

 OR

RN

• Active Indiana/Kentucky RN license or valid compact RN license 

Shift: Monday-Friday; Days

Hours: 32 per week

Pay Ranges:

BSW- $22.32-$31.25

MSW- $26.04-$36.45

RN- $31.57- $50.25
