PAY RATE: $45.00 – $56.00 per hour, depending on experience, plus shift differentials
SHIFT DIFFERENTIALS:
3–11 PM: +$1.00/hr 11 PM–7 AM: +$1.00/hr Weekends (All Shifts): +$1.50/hr
POSITION OVERVIEW:
Mystic Healthcare & Rehabilitation Center is seeking an experienced and compassionate Registered Nurse (RN) to join our clinical team. In this role, you will lead resident care delivery, oversee LPNs and CNAs, collaborate with physicians and interdisciplinary staff, and ensure that our residents receive safe, high-quality, person-centered care in a long-term care and rehabilitation environment.
KEY RESPONSIBILITIES:
- Conduct comprehensive nursing assessments and develop individualized care plans
- Administer medications, IV therapies, and treatments per physician orders and facility protocols
- Monitor residents for changes in condition and initiate appropriate clinical interventions
- Supervise and provide clinical direction to LPNs, CNAs, and support staff on the unit
- Communicate with attending physicians, specialists, and care team members regarding resident status and care needs
- Coordinate admissions, discharges, and transfers, including completion of required clinical documentation
- Perform and oversee skilled nursing procedures including wound care, tube feedings, catheter care, and more
- Respond to emergencies and serve as clinical lead during urgent situations
- Ensure compliance with state and federal long-term care regulations and facility policies
- Maintain accurate, timely, and thorough documentation in the electronic health record
- Educate residents and families on care plans, conditions, and discharge instructions
- Participate in quality improvement initiatives and care conferences
QUALIFICATIONS & REQUIREMENTS:
- Active Connecticut RN license in good standing (required)
- Associate's or Bachelor's degree in Nursing from an accredited program (BSN preferred)
- Long-term care, skilled nursing, or rehabilitation experience preferred
- CPR/BLS certification (or willingness to obtain upon hire)
- Strong clinical assessment, critical thinking, and leadership skills
- Ability to supervise and mentor LPNs and CNAs effectively
- Excellent written and verbal communication skills
- Ability to stand, walk, and lift up to 50 lbs for extended periods
- Experience with electronic health records (EHR) preferred
