Job Summary:

The RN Navigator is a Registered Nurse whose clinical nursing experience will ensure successful medication management for patients being treated for chronic illnesses. Monitors patients'' progress throughout treatment. Intervenes as needed to ensure compliance and optimum care. Collaborates within a multidisciplinary team to facilitate timely, appropriate screening, therapeutic intervention, treatment and supportive care across the health care continuum. The RN Navigator provides patient education for each patient regarding their disease process and treatment throughout the health care continuum. Demonstrates and promotes professional development. Maintains a professional, safe, and therapeutic environment for rendering care. This RN will also complete new patient intakes for patients newly diagnosed with HIV, transfers of care for patients living with HIV, Hep C, and post-exposure patients (PEP) with exposure to bodily fluids. Ensure follow-up and intake with new patients, Hep C patient and PEP patients to ensure medication compliance and provide education and referral to higher levels of case management, if needed.

Minimum Qualifications:

NYS licensed/registered (or eligible) RN required. Bachelor''s degree in Nursing or healthcare-related field, three (3) years related RN experience required OR an Associate''s degree in Nursing and five (5) years related RN experience required. Excellent written/oral communication and interpersonal skills required. CPR-BLS certification from AHA / Red Cross is required by the time of appointment.

Preferred Qualifications:

Prior ambulatory or medication management experience is preferred. Experience working with HIV + and Hep C + patients.
