    Job Description

    Please note: NSORNA Nisqually's Sex Offender Registration and Notification Act requires all individuals who have been convicted of a sex offense to register with our Public Safety Department. This is true whether or not you have to register with any other jurisdiction.

    ICW/Vulnerable Adults. This position has regular contact with or control over Indian Children and Vulnerable Adults. In compliance with Federal Law, this position will not be filled by an applicant with any felonious offense or any of two or more misdemeanor offenses under Federal, State, or Tribal Law involving crimes of violence; sexual assault, molestation, exploitation, contact, or prostitution; crimes against persons; or offenses committed against children.

    *This position requires a fingerprint check, which is a standard procedure for many jobs, especially those involving children or vulnerable populations.


    GENERAL SUMMARY

    TheNurseCare managerwillworkcloselywith providerteamstosupportpatientsin the spirit of the mission and vision of the Nisqually Generations Healing Center. The focus of this position will be Integrating primary care and chronic disease managementwithintheOpioidTreatmentProgram (OTP) setting.

    This position ensures confidentiality, security, and accuracy of records, and promotes positive employee, customer, and community relations. Strong written and verbal communication skills are required and outstanding customer service to fellow employees, customers, vendors, etc. is expected. Regular attendance is required, later hours or weekend time may be required, and a neat we/I-groomed professional appearance is essential at all times.


    PRIMARY RESPONSIBILITIES

    • Conductscomprehensiveassessmentof patients,Includingacute and/or chronic health problems.
    • Works with a multi-disciplinary team providing targeted care management, education, and training services to patients and staff.
    • Assistswiththeidentificationofpatientswithspecialhealthcareneeds/chronic diseases.
    • Carriesoutcareplans,evaluates,monitorsona regularbasis,and effects changes as needed.
    • Developsmethodoftracking patientcompliancetoclinicalguidelinesandplanof care
    • Triagestelephonecallsfrom patients.
    • Providesdirectwoundcarefor patients as needed.
    • Ensurespatientconfidentiality asdefinedby HIPAAand the medical facility.
    • Records and documents health information necessary to maintain, coordinate and /or expedite required servicesandqualityimprovement programs.
    • Develops, conducts, and evaluates formal or informal in-services and educational programs in area of expertise.
    • In support of the research process, contributes ideas, collects data, shares knowledge, and applies new findings to practice.
    • Participates as a team member in the provision of medical and health care, Interfacing with professional colleaguestoprovidecomprehensiveIsencouragedtoparticipateoncommitteesandattend requiredmeetings,andin-services.
    • Developsandteachesrelevantdiseaseeducationclassesas required.
    • Monitorsforregulatory compliance.
    • Documentsphone callsandfacetoface encounterswithpatients,aswell as submitbilling documentation and coding in a timely manner to ensure proper reimbursement of services provided.
    • Practices universal blood and body fluid precautions, appropriately handles hazardous materials, and followsOSHA regulationsperapproved policiesand procedures.
    • Developandmaintainclientfliestomeetalllocal,state,andfederalprogram requirements.
    • Ensure securityandconfidentialityofallprogram dataandoperateinaccordancewithapplicable regulationsofHealthInsurancePortabilityand AccountabilityAct(HIPAA) and42CFR Part 2.
    • Performs other duties as assigned.

    MINIMUM QUALIFICATIONS

    • Must be a Licensed Registered Nursein good standing withtheState of Washington.
    • Fiveyears'experienceofprofessionalworkasaRegisteredNurseis desirable.
    • Must possessa validWashingtonState Driver'slicenseandmaintaina driving recordthat meetsthe minimumrequirementsestablished byNisqually IndianTribes vehicleinsurance.
    • Successfullypassa pre-employment drugscreenandcriminalbackground.
    • Theabilitytowork withvulnerablepopulations including adultsand children.
    • Must pass a state and national (FBI) criminal history background check prior to employment.

    PREFERRED QUALIFICATIONS

    • Previoussubstanceabuse treatmentand/orprimarycare experiencestrongly preferred.
    • Knowledge of the Nisqually Indian Tribe's history, customs, traditions, and ability to demonstrate cultural sensitivity.
    • Experience working with Federally Recognized Tribes.

    SUPERVISION

    This position trains, supervises, and evaluates staff.

    • Knowledgeof the current medical practices in primary care, mental health and addiction medicine. Management of sexually acquired Illnesses, Hep C, family planning and familiarity with prenatal care.
    • Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and This includes symptoms, treatment alternatives, drug properties and Interactions, and preventive health-care measures.
    • Principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
    • Tribal,Federal,and state laws,codes,and policies.
    • HIPAA and PHI Knowledge and adherence to state (RCW, WAC) and federal confidentialityregulations(42CFRpart2)
    • Commonofficeandadministrative duties.
    • Trauma and its impact on wellness and recovery.
