A Path of Care is seeking a full-time RN Nurse Navigator in our Tulsa, OK location who balances expert clinical judgment with the heart of an advocate. This isn't just an administrative role-it's a dynamic, field-based position where you act as the bridge between the hospital, the physician's office, and the patient's home.

In this role, you aren't just processing paperwork; you are on the ground visiting hospitals, rounding with physicians, and meeting families in their most vulnerable moments to ensure they receive the high-quality care they deserve.

What You'll Do

  • Clinical Roadmap Specialist: Perform deep-dive clinical reviews of referrals to determine the best path for care-whether it's rehabilitative home health or compassionate hospice.
  • Field Liaison: Conduct onsite visits to hospitals and facilities to meet potential patients, easing their transition and ensuring no detail is missed.
  • Physician Partner: Join rounds with interdisciplinary teams to provide clinical insights and ensure everyone is aligned on the patient's goals-of-care.
  • Empowering Educator: Serve as a lifeline for families, teaching them about disease progression, medications, and what to expect during the care journey.
  • Advocate for Independence: Support advanced care planning and identify clinical risks to help reduce hospital readmissions.

Qualifications

  • Experience: A minimum of 2-3 years in clinical nursing (Home Health, Hospice, Case Management, or Acute Care preferred).
  • Licensure: An active Oklahoma Registered Nurse (RN) license is required.
  • Skills: You are a critical thinker with strong assessment skills and a deep understanding of Medicare eligibility guidelines.
  • Heart: You possess the "soft skills" necessary for end-of-life discussions and the "hard skills" needed for accurate, timely documentation.

Schedule

This is a full-time position, Monday-Friday, 8:00 AM - 5:00 PM (40 hours per week).
