Patient navigation in cancer care refers to individualized assistance offered to patients, families, and caregivers to help overcome health care system barriers and facilitate timely access to quality medical and psychosocial care. This assistance may be provided pre-cancer diagnosis (screening and education) through continued phases of the cancer experience. The navigation services implemented will depend upon the particular type, severity, and/or complexity of the identified barriers. This job exists to facilitate a timely, integrated and coordinated approach to the care of patients with a diagnosis of cancer and to provide quality nursing care for patients with cancer related health issues. The mission of the Oncology Nurse Navigator (ONN) is to improve patient preparedness for treatment by providing education and psychosocial support. The ONN also facilitates interaction between patients and their physicians provide logistical support, referrals, coordination of care, and other assistance during the trajectory and transitioning points of the patient's oncology care. The ONN will serve as an advocate to assure positive customer satisfaction. The ONN will use a multi-disciplinary approach to planning and providing care and act as a liaison to all other areas of the hospital to insure that the needs and concerns of oncology patients and their families are addressed. An oncology nurse navigator (ONN) is a professional registered nurse with oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process, an ONN provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum
