Join our dynamic team as a Registered Nurse specializing in Case Management for individuals with developmental disabilities within a supportive group home environment. This full-time role offers a flexible schedule from Monday to Friday during daytime hours, allowing you to balance work and life effectively.

Key Responsibilities

• Ensure the well-being of residents by diligently updating and maintaining their monthly Medication Administration Records (MARS) and health records, in accordance with OPWDD regulations.
• Coordinate and manage medical appointments for individuals, ensuring timely follow-ups and comprehensive care.
• Proactively request new medical orders or extensions on existing ones, ensuring no lapse in the care plan.
• Be a critical point of contact for urgent health concerns, providing triage services as needed while present in the group home.
• Conduct thorough body checks and assessments, documenting findings through detailed nursing notes to ensure continuity of care.
• Play a pivotal role in supervising medication passes for AMAP Certifications, ensuring safe and effective medication management.
