Job Requisition Title: RN Patient Care Coordinator/Case Manager
Location: Hillsboro and Portland, OR 
Shift: 08:00 AM to 05:00 PM
Pay: $Best in job marker

Description:

Oregon & WA RN or compact license active and on file
Must have 1 year of case manager or PCC experience
RN will be needed to work on wknds Sun-Sat w/holidays
Previous KP experience preferred
EPIC charting and experience with Medicaid/Medicare
