About this position

The Patient Care Facilitator (PCF) proactively directs customer care from shift to shift to ensure customers receive an exceptional experience of care. Primary accountabilities include care coordination, quality, and customer and family satisfaction. The PCF effectively allocates resources by assuring appropriate customer assignments, staffing and scheduling and provides direct customer care as needed. Actively and consistently demonstrates the mission, vision, values and guiding principles of Maple Grove Hospital.

The Intensive Care Center is a unique and dynamic unit grounded in strong teamwork and collaboration. While certain hospital services are limited-such as the absence of dialysis and a cath lab, along with reduced IR availability-our patient population reflects these boundaries while still offering a broad range of clinical experiences.

We care for a diverse mix of medical and surgical patients, including pulmonary, cardiac, gastrointestinal, genitourinary, endocrine, and elective surgery populations. We also manage a select group of intensive care patients who require higher‑level interventions, such as vasoactive infusions and mechanical ventilation. On average, our census includes approximately two critical care patients per day.

While the unit supports both medical/surgical and intensive care-level care, the majority of daily assignments will be med/surg focused, with ICU‑level assignments occurring as needed based on patient acuity and census. This blend of patient types and acuity levels creates a versatile, collaborative, and rewarding clinical environment for our team.

SPECIFIC ESSENTIAL DUTIES:

  • Recognizes Cardiac, Respiratory, Neurologic, Renal, Endocrine, and GI/GU risk factors,

monitors for changes in a critical patient status, and seeks out the appropriate

interventions by collaborating with ancillary departments and care team members to meet

the needs of the patient.

  • Transfer of critical patient to another facility for higher level of care: Level 1 Heart, Level 1

Stroke

  • Monitors and manages advanced airways/respiratory adjuncts including but not limited to:

ET tube insertion/maintenance and tracheostomy site care. Collaborates with Respiratory

Therapy when providing care for patients requiring a ventilator/bipap/cpap support or has

oxygen therapy needs.

  • Monitors and manages cardiac conditions including but not limited to: arrhythmia

management, unstable heart rate and/or blood pressure management.

  • Monitors and responds/notifies members of the care team in regards to critical laboratory

findings including but not limited to: electrolyte imbalances, chemistry/blood counts,

abnormal blood gas results and/or drug levels.

  • Monitors and intervenes for high-risk patient populations including but not limited to:

Stroke, MI, Drug/Medication Overdose, Shock, Sepsis, Massive & Emergent Blood

Administration, and Multiple organ dysfunction syndrome (organ failure).

  • Monitors and manages a patient at high-risk for self-harm including: suicidal patients,

medical/behavioral holds, patients in restraints, ETOH/drug withdrawal. impaired

mobility/falls risk.

  • Initiates and monitors high risk medications (continuous infusions, bolus, IV push, and

titrations). This includes the monitoring of hemodynamics/vital signs, the patients'

response to treatment, and laboratory results.

  • Manages the set-up, equipment/tubing changing and discontinuation of hemodynamic

lines including: Central Lines/CVP, Arterial Lines, and/or a Pulmonary Artery Catheter.

  • Understands and manages the critical patient requiring invasive hemodynamic monitoring

including: Interpretation of waveform readings, troubleshooting equipment issues and

notifying the care team provider with changes in the patient condition.

  • Identifies and provides treatment (antidote if applicable) for an IV Extravasation and for

Central Line occlusions.

  • Locates and initiates essential critical care equipment as a situation warrants: Cardiac

monitor, Crash Cart (ACLS supplies/medications, airway supplies), Defibrillator/AED,

Meditherm Cooling Blanket, Central Line Cart.

  • Sets-up and assists with invasive critical care procedures including but not limited to:

chest tube insertion, intubation, thoracentesis. central line placement.

  • Administers and monitors patients requiring Procedural/Conscious Sedation. This

includes the completion of required documentation, medication administration and the

recovery of the patient.

Requirements

Education

  • Associates Degree in Nursing; Bachelor's Degree in nursing preferred.

Experience

  • 3-5 years nursing experience in a hospital setting. Previous experience in a leadership role preferred.
  • Demonstrated ability to maintain composure in high stress situations; acts quickly in response to critical situations.
  • Previous experience and participation in care coordination and quality management activities preferred.
  • Solid PC skills to include electronic medical record, MS applications, etc.
  • Strong communication skills with demonstrated ability to express ideas and information (both in written and oral form), clearly and concisely in a manner appropriate to the audience.
  • Demonstrated ability to deal with conflict in a positive manner. Has an awareness of, responds to, and considers the needs, feelings and capabilities of others.
  • Ability to be accountable for quality customer service and respond appropriately to a variety of personalities and situations.
  • Ability to maintain quality, safety, and infection control standards.
  • Must be able to read, write and speak effectively in English.

SPECIAL KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

  • Knowledge of nursing theory and practice and primary care principles and practices.

Licensure/Certification

  • Current licensure and registration as an RN in the State of MN.
  • Current BLS certification for all areas of nursing practice required.
