RN Patient Care Manager

In this impactful full-time role, scheduled Monday through Friday, from 8 a.m. to 5 p.m., you will ensure exceptional patient care by leading and mentoring a team of clinical professionals. Your responsibilities include overseeing clinical practices, coordinating patient services, and managing staff to maintain consistent, high-quality care delivery. You will serve as a pivotal communication link between patients, caregivers, and various departments, actively driving quality improvements and organizational goals. Compliance and safety are at the forefront of this position, requiring adherence to training and reporting protocols and promotion of safety standards. Ideal candidates are registered nurses with active state licensure and have previous leadership experience in a home care setting. A thorough understanding of best practices, federal and state regulations, and Medicare guidelines is essential.
