RN Patient Care Manager

As the leader of our clinical field team, you will play a crucial role in overseeing the quality and effectiveness of patient care, ensuring that it aligns with professional standards and complies with all relevant regulations. You will coordinate the delivery of care, supervise and mentor clinical staff, and maintain effective communication with patients, families, and healthcare partners. By driving improvement initiatives and upholding safety practices, you’ll contribute to the consistent achievement of quality goals. You will also foster professional growth by providing coaching and identifying continuing education opportunities. Success in this role requires a valid state nursing license, experience in a home care environment, and a depth of understanding of Medicare and regulatory compliance.
