Assessment
Performs an appropriate nursing history and physical upon admission.
Reviews complete physical assessment
Performs frequent vital signs
Identifies any problems based on observation techniques
Analyzes all data and incorporates into plan of care
Compares and correlates data in written record
Utilizes standards of care and previous clinical information to anticipate potential patient care problems
Identifies individual patient needs
Reviews pre-procedure history, physical examination, medical care plan and documented allied health care (i.e., R.T., Xray, Social Services)
Identifies abnormal diagnostic data, records and communicates to appropriate resource.
Performs on-going assessments related to plan of care on patient condition as evidenced by random audit.
Planning
In conjunction with the patient and family, develops individualized patient care plan based on nursing diagnosis. Identifies short and long term goals, including discharge planning, consistent with established patient care plan
Identifies and documents expected outcome relevant to specific nursing interventions as evidenced by random audit.
Identifies and reports unsafe practice and/or procedure accurately and timely to ensure patient safety.
Checks and maintains assigned rooms with 100% compliance to ensure adequate supplies and equipment are available for patient care as evidenced by random audit.
Knows location, purpose and use of all equipment and supplies and conserves use of supplies to ensure cost effective, quality care.
Implementation
Provides direct nursing care based on the plan of care and established priorities.
Performs and documents nursing care given based on nursing diagnosis as evidenced by random audit.
Performs and documents nursing care and patient education based on accepted standards/protocols in a safe manner.
Record pertinent information clearly, accurately and in a timely manner as evidenced by random audit.
Adapts care in response to alterations in patients needs and/or behavior.
Control of Medications
Accurately administers medications per hospital policies.
Accurately charts medication.
Accurately administers IV therapy per hospital policies.
Teaching Skills
Implements the teaching plan.
Evaluates patient's learning by observing follow-through and/or oral feedback
Contributes to positive learning experiences of nursing students/staff.
Participates in teaching and guiding the performance of paraprofessional personnel.
Communication
Applies effective interviewing skills to elicit information from patient and/or significant others
Communicates accurate information about the patient care plan to patient and/or significant others.
Identifies and reduces anxiety in patient and/or significant others through verbal and non-verbal communication
