Compensation

Pay Range: $40.07 - $48.10 per hour, depending on prior experience.

What You Bring (Requirements)

• Graduation from an accredited nursing program; a Bachelor's degree is preferred.
• Current RN license in the state of Colorado.
• At least two years of nursing experience, including long-term care, dementia care, or acute care; home care experience is a plus.
• CPR certification.

Your Responsibilities

As a Registered Nurse (RN), you will:

• Manage cases, coordinate care, supervise, and implement professional and supportive services for clients, utilizing nursing theory and processes in line with the Nurse Practice Act.
• Review client referral information and respond to inquiries as necessary.
• Conduct client assessments according to agency standards to ensure effective home care services.
• Independently direct, plan, and initiate appropriate actions in home care situations.
• Perform duties in accordance with agency policies, procedures, and professional standards.
• Complete additional tasks as assigned by the Director of Clinical Services.

We have a dedicated team of nurses, rehabilitation therapists, and home health aides committed to providing personalized healthcare with compassion and empathy. We specialize in caring for older adults and offer in-home health services across Delta, Montrose, and Ouray counties.

Experience Required

• 2 years of nursing experience, including a mix of long-term care, dementia care, or acute care.
• Preferred: Experience in home care.

Education Preferred

• Bachelor's degree in Nursing or a related field.

Licenses & Certifications Required

• CPR/First Aid certification.
• Registered Nurse license.
