Responsibilities/Job Description:

Fairview Health Services is searching for a hardworking RN who is looking for an exciting career in our Cancer Clinic in Burnsville.

 

This position is approved for 80 hours every two weeks.  We believe the schedule will be Monday - Friday 8:00 am to 4:30 pm 

 

The Registered Nurse is responsible for the assessment, planning, delivery, and evaluation of nursing care for all assigned patient population(s). The RN is responsible for performing these responsibilities in accordance with all policy, procedures, professional practice guidelines and the MN Nurse Practice Act. The RN coordinates care with members of the health care team and delegates responsibilities to others as appropriate.

Responsibilities

  • Assesses patient needs and identifies expected nursing outcomes.
  • Collaborates with patient, family and other health team members to develop a nursing plan as part of an integrated, comprehensive plan of care
  • Develops an individualized plan based on age, social and cultural considerations, condition and other relevant needs.
  • Evaluates care and patient response to interventions and expected outcomes.
  • Collaborates and communicates effectively and respectfully with the patient, family and members of the health care team within and across care settings to promote partnership-based health care
  • Demonstrates the ability to advance care by utilizing standards, guidelines, care pathways and continuous improvement to enhance patient outcomes.
  • Promotes a culture of evidence based practice (EBP)
  • Coordinates care for patients and families in collaboration with the multidisciplinary team across the continuum of care with an emphasis on newly diagnosed patients.

    • Synthesizes assessment data and resulting diagnosis. Plans interventions and expected outcomes to provide patient & family necessary information.
    • Serves as clinical expert/resource to oncology patients, families, health care team and community.
    • Assists the patient in coordinating multiple specialist appointments, and provides education for patients.
    • Coordinates multidisciplinary team conferences.
    • Communicates with referring health care providers regarding comprehensive plan of care for patient and family.
    • Works closely with physicians and departments to deliver coordinated care in a timely manner.
    • Works with multidisciplinary team to develop quality monitors to measure patients’ outcomes and program performance.


Required Qualifications

  • MN Registered Nurse (RN) License
  • Basic Life Support (American Heart Assoc or Red Cross)
  • Minimum of 1 year working in oncology nursing. 

Preferred Qualifications

  • Bachelors Degree in Nursing or BAN
  • 1-3 years of experience
  • 1 year of care coordination

$44.68- $75.66 Hourly
