Elderwood at Cheektowaga is seeking Registered Nurses to join our skilled nursing and rehabilitation team. This is more than a traditional long-term-care nursing position. Our RNs care for long-term residents and short-stay rehabilitation patients transitioning from hospital to home, creating an environment with meaningful clinical variety, interdisciplinary collaboration, and opportunities to strengthen your assessment and technical skills.
You will provide direct nursing care, coordinate with providers and rehabilitation professionals, guide CNAs, respond to changes in condition, and develop relationships with residents and families that often extend beyond a brief hospital encounter.

Elderwood at Cheektowaga is a 172-bed skilled nursing community providing long-term care, memory care, and comprehensive subacute rehabilitation.
Our clinical teams care for individuals recovering from illness, injury, surgery, orthopedic procedures, and cardiac conditions. Facility capabilities include:
IV therapy
PICC-line management
Wound-vac therapy
Complex wound care
Ostomy care
Enteral feeding
Dialysis coordination
Depending on your assignment, resident needs, physician orders, and demonstrated competencies, you may care for patients receiving these treatments while managing medications, completing assessments, monitoring clinical progress, coordinating services, and preparing patients for a safe transition to their next level of care.
For RNs who enjoy med-surg-style clinical variety but also value continuity, teamwork, and meaningful patient relationships, this is an engaging post-acute nursing environment.
Registered Nurses at Elderwood have meaningful clinical responsibility - but they are not expected to practice without support. You will work with onsite nursing leaders, including Unit Managers, RN Supervisors, the Assistant Director of Nursing, and the Director of Nursing. You will also collaborate with providers, rehabilitation professionals, and other members of the interdisciplinary team.
Facility clinical leadership receives additional support from Elderwood Administrative Services, including organization-wide direction from the Chief Nursing Officer and an assigned Regional Nurse Consultant. This broader structure supports clinical standards, quality, regulatory readiness, leadership development, and problem-solving throughout the facility—giving bedside nurses the benefit of working within an established clinical organization.

What You’ll Do as a Registered Nurse (RN) at Elderwood:
Assess, monitor, and evaluate the clinical status of assigned residents and short-stay rehabilitation patients
Administer medications, treatments, and other ordered nursing services safely and accurately
Identify and respond promptly to symptoms, risks, and changes in condition
Communicate clinical changes and concerns to providers, nursing leaders, residents, and families
Follow individualized plans of care and contribute information needed to keep those plans current
Support admissions, care transitions, rehabilitation goals, and discharge planning as assigned
Maintain accurate nursing notes, medication-administration records, treatment records, and other required documentation
Collaborate with providers, rehabilitation professionals, social work, dietary, pharmacy, and other members of the interdisciplinary team
Coordinate care and follow-through for residents with complex treatments and appointments
Help ensure that physician orders, treatment plans, and clinical recommendations are implemented appropriately
Support safe communication during shift report and transitions of care
Educate residents and families regarding care needs, treatments, medications, and recovery goals within the RN’s role
Direct and monitor personal-care duties and nursing procedures completed by CNAs on the assigned team
Establish clear expectations and provide guidance throughout the shift
Assist CNAs with direct resident care when resident or unit needs require it
Monitor implementation of each resident’s plan of care
Promote respectful communication, teamwork, reliability, and accountability
Escalate staffing, clinical, safety, or performance concerns to the appropriate nursing leader
Protect resident rights, dignity, preferences, confidentiality, and independence
Support infection prevention, medication safety, fall prevention, skin integrity, and other quality priorities
Maintain readiness for regulatory surveys and clinical-quality reviews
Respond to resident and family concerns professionally and with appropriate follow-through
Contribute to a safe, organized, respectful, and resident-centered care environment
What You’ll Need as a Registered Nurse (RN) with Elderwood:
Current license and registration to practice as a Registered Professional Nurse in New York State
Ability to assess residents, administer medications and treatments, respond to changes in condition, and document nursing care accurately
Ability to direct, support, and work collaboratively with CNAs and other members of the care team
Strong verbal and written communication skills
Sound clinical judgment, organization, and problem-solving abilities
A caring, mature, diplomatic, and respectful approach to residents, families, and coworkers
Physical stamina for an active, hands-on nursing position
Ability to read, write, speak, and understand English at the level necessary for safe care and effective workplace communication
Nursing experience in skilled nursing, subacute rehabilitation, long-term care, geriatrics, medical-surgical nursing, or another clinically complex setting
Experience caring for residents with dementia or cognitive impairment
Experience with IV therapy, PICC lines, wound care, enteral feeding, dialysis coordination, or post-acute recovery
Experience directing or supporting CNAs as part of a nursing team
Nurses from hospital, rehabilitation, medical-surgical, dialysis, home health, assisted living, and other clinical settings are encouraged to apply. Previous skilled nursing experience is valuable, but clinically capable RNs interested in developing post-acute expertise should also be considered.
