About the Health Care Quality Unit (HCQU)

The Health Care Quality Unit (HCQU), operated by Geisinger in partnership with the Pennsylvania Department of Human Services and the Office of Developmental Programs, supports Pennsylvania's intellectual disability and autism (ID/A) community. Through training, education, and resources, the HCQU helps improve health, wellness, and quality of life for individuals with intellectual and developmental disabilities and autism while supporting county ID/A programs and service providers.

In this role as the Health Care Quality RN, your patient population of focus will be individuals with intellectual/developmental disabilities and autism living in community settings. 

  • At least three (3) years of RN work experience is required.

  • Preferred candidates will have experience working as an RN with individuals with intellectual and developmental disabilities and autism who are living in community or residential care settings.

  • This role is full-time, 40 hours weekly.

Position Details

You'll coordinate and promote quality healthcare services for people with intellectual/developmental disabilities and autism living in community settings. through the provision of health-related training, clinical expertise, and technical assistance. Your goal is aimed at building and expanding the person's competency and capacity in the health care area. Facilitate access to and coordination of appropriate health care services.

  • Create global awareness of stake holders throughout the behavioral health system on physical health and behavioral health issues Build linkages to community and social supports to enhance coordination of medical and behavioral health and existing network of providers.
  • Identify individuals who have extensive co-morbid physical and behavioral health conditions; Analyze individual and aggregate physical and behavioral health care utilization data Managed Care Organizations on these individuals;.
  • Identify and provide- whenever possible through Case Managers-outreach to individuals who are over utilizing behavioral and physical health care services and offer them assistance in developing an individualized wellness plan.
  • Design and conduct periodic health surveys for community members with co-morbid conditions; Provide case-specific training and consultation to behavioral health staff (and others who are working directly with clients with co-morbid physical health and behavioral health conditions;.
  • Develop and implement programs around compliance to medication and medical appointments.
  • When necessary, accompany clients to medical appointments to ensure they are getting optimal services and provide follow up consultation and monitoring to ensure optimal compliance with providers' recommendations Plan and secure web-based and face-to-face trainings for behavioral health staff and consumers on common health-related conditions.
  • Advocates for the health of the consumer by referring and facilitating access to quality physical and behavioral health services intended to meet individualized needs; facilitating access and coordination of behavioral and physical health care services assist and coach clients to advocate for themselves in meetings with health care providers.
  • Develop preventative wellness plans for clients new to the system to preclude the development of chronic health conditions i.e. weight management, nutrition, health screenings, exercise, and smoking cessation; communicates regularly with individual consumers, caregivers, families, county representatives and local health care providers to establish priorities, identify training opportunities, and identify problem areas and needs.

Education

Graduate from Specialty Training Program-Nursing (Required), Bachelor's Degree- (Preferred)
