We are currently looking for the right RN to take care of a male in Somerville. He needs 1 hours visits 12p-1p (flexible) Monday through Saturday for wound care.

What You'll Do (Job Responsibilities):
- Provide high-quality, patient-centered nursing care by assessing, planning, implementing, and evaluating patient needs in the comfort of their homes.
- Collaborate closely with a multidisciplinary clinical team to optimize patient outcomes.
- Educate patients and their families on health maintenance, medication management, and disease prevention.
- Maintain accurate, timely electronic documentation in compliance with state and federal regulatory guidelines.
What We're Looking For (Requirements):
- License: Active, unrestricted Registered Nurse (RN) license in the State of Massachusetts.
- Experience: Previous experience in home health, hospice, or an acute care setting is highly advantageous (New grads with a passion for home health are welcome to apply!).
- Skills: Exceptional interpersonal and communication skills with a focus on empathy and clinical excellence.
- Vehicle: Reliable transportation and valid driver's license for travel to patient homes.
