Common diagnosis / Types of patients/patient demographics No Peds. Most of our patients are recently discharged from the hospital and are at a high risk for rehospitalization. CHF/ COPD/ wounds/some wound vacs/ IV-PICC lines Additional Job Duties/Utilization Review involved? RT Equipment (vents, Cpap, BiPap, etc) Only related to typical patient education Experience or 'Must have' skills HCHB- HomeCare HomeBase preferred How many patients do they see per day? 5.25 points per day Start of care 2.5 pts/ recert 1.5 pts/ resumption of care 1.75 pts/ and routine visits1 pt
This is usually 5 visits per day unless start of cares are scheduled- since they have a higher point value Manager/Supervisor Holly Haynes- Available during working hours for routine questions and after hours for urgent needs. Administrator on call available after hours and on weekends. Home Health/Hospice Aids? We do have a home health aide in our department Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?) 5 days a week/ 8 hour shifts. We try to have the schedule completed by the previous day, but there are common changes to the schedule in the morning. Ex: SOC that are not completely processed or sick calls. Weekend Requirements I am flexible- shift can either be Monday -Friday or Tuesday-Saturday (I would prefer Tuesday-Saturday) Holiday Requirements Yes- I could use help Thanksgiving and/or Christmas Is there a call or standby requirement?
