Job Description

Pay Rate: $56 - $58 per hour

Shift: NIGHTS 7:00pm-7:30am, EOW

Summary:

  • Deliver patient care by incorporating the tenets of the professional practice model of Relationship Centered Care.
  • Apply the nursing process based on theory, research, evidence-based practice, and approved organizational nursing standards.

Responsibilities:

  • Assess and evaluate patient care needs using critical thinking skills.
  • Integrate relevant assessment and intervention skills in the delivery of nursing care.
  • Collaborate with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
  • Prioritize all aspects of patient care, including teaching, rounding, coaching, and planning “after hospital care.”
  • Educate patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease.
  • Communicate effectively and professionally with patients, family, and all members of the Interdisciplinary Patient Care Team.
  • Manage assignments within the Care Delivery Model of Modified Primary Nursing.
  • Role model the tenets of Relationship Centered Care through various patient-focused actions.
  • Establish and maintain a therapeutic relationship with the patient and family.
  • Initiate service recovery for patients and family members who have concerns.
  • Practice safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement.
  • Collaborate with interdisciplinary teams to create and implement quality improvement projects.
  • Perform charge nurse duties as assigned.
  • Coordinate with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager to ensure supplies are appropriately utilized.
  • Facilitate patient throughput on shift basis.
  • Contribute to the environment of care to support clinical colleagues, patients, and their families.
  • Advance clinical competence in nursing practice.
  • Encourage and demonstrate a spirit of scholarship, inquiry, life-long learning, and innovation for self and others.

Requirements:

  • NYS RN License
  • BLS certification
  • Minimum 2 years of rehab & mechanical ventilation experience
  • Minimum 2 years RN experience

Preferred Skills:

  • EPIC experience highly preferred
