This role cares for short-term rehabilitation residents and long-term residents with complex, chronic, and age-related conditions. The Registered Nurse – Skilled Nursing Facility establishes authentic, personalized relationships with residents and their support systems, collaborating with physicians and an interdisciplinary team—including nursing assistants, rehabilitation therapists, and social services—to assess, plan, implement, and evaluate individualized plans of care.
The Registered Nurse – Skilled Nursing Facility ensures compliance with CMS regulations and state and federal guidelines, contributes to the interdisciplinary care planning process including Minimum Data Set (MDS) assessments, and maintains survey readiness.
