RN Staff Nurse

Careway Medical Adult Day Care, a modern, attractive health care facility in Elmwood Pk, NJ, is seeking a full-time or part-time registered nurse (with a valid NJ nursing license) to join our team of dedicated health care professionals delivering services to our culturally diverse population of clients. The registered nurse serves as part of our multidisciplinary team of employees who share the common goal of keeping at-risk individuals safe in their communities.

Responsibilities:

• Maintaining communication and relationships with clients ' physicians and caregivers to ensure continuity of care
• Documenting initial and quarterly assessments and daily trigger notes
• Facilitate referrals to other healthcare professionals and medical facilities
• Monitor and document vital signs
• Follow physician's orders to maintain an optimal level of functioning
• Developing monthly care plans
• Educate patients about health maintenance and disease prevention
• Assisting clients with ADL's as needed

Qualifications

• RN license required
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Knowledge of electronic health records helpful
• Knowledge of Spanish is helpful but not necessary.

No Weekend Hours or on-call hours. The position is solely Monday-Friday

Our Medical Adult Day Care provides a warm and home like setting for older adults who need monitoring of their health conditions. Participants in our program come from culturally diverse communities and spend the day visiting with their peers, attending activities geared toward their individual interests and abilities, and receiving follow up attention for their health issues. Our mission is to provide the structure and support needed so that at risk older adults can remain and thrive in their community settings.
