Position: RN Stroke Program Coordinator
Location: Elmhurst, IL
Full Time: 40 hours / week
Office Hours: Monday - Friday 8:00 AM - 4:30 PM.
What you will do:
Responsible for stroke program oversight; responsibility and accountability for the stroke program and provides clinical and administrative leadership in the care of stroke patients through the care continuum. Coordinates and evaluates operations and clinical program processes to ensure the delivery of quality patient care throughout the hospital. Assists the System Director with establishing the goals for the program to provide patient focused quality care, patient satisfaction and cost-efficient care. Direct oversight and responsibility for stroke program regulatory accreditation. Responsible for the day to day review of Stroke (Ischemic/Hemorrhagic/TIA) and Neuroscience patients. Responds and coordinates response of others to Code Strokes in the hospital/ED. Recognizes trends in stroke care and explores stroke/neurosciences care process improvements. Assists in the development and revision of standards of care/guidelines/clinical pathways/order sets for stroke/neuroscience patients, incorporating an interdisciplinary approach, utilizing best practices, and research findings. Responsible for collecting and recording data required to achieve the benchmark targets, specifically STK Advanced Primary Stroke Center measures. Monitors compliance with regulatory guidelines, quality guidelines, and Advanced Primary Stroke Center guidelines. Works with nursing, rehabilitation and other services to ensure comprehensive patient education and preparation is achieved and maintained, on an ongoing basis. Assists in coordinating care of the Stroke/Neuroscience program patients through collaboration with physicians, the stroke coordinator, nursing, case management, social workers, therapists, radiology, lab, dieticians, pharmacy, and all other members of the healthcare team through team meetings and/or one-on-one meetings to ensure a quality care-delivery process for patients across the continuum. Participates in staff education on an individual and/or group basis. Provides support to the Stroke/Neuroscience team for continual learning. Coordinates with educators in the development of education programs for staff orientation, annual education, and yearly competencies.
What you will need:
Current Illinois RN licensure required
Bachelor's Degree required
Current CPR Certification issued by American Heart Association (AHA) required
Preferred 3 years + of RN Neuro/Stroke related experience
