Job Summary:
Tryfacta is seeking a RN Supervisor Long Term Care (LTC)for our client in Vestal, NY 13850 . This is a 16-week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title: RN Supervisor Long Term Care (LTC)
Location: Vestal, NY 13850
Duration: 16 Weeks (Possible Extension)
Work Schedule: Day 5x8-Hour (06:30 - 15:45)

Responsibilities for this position include, but are not limited to:
  • Supervise and coordinate the daily activities of nursing staff, including RNs, LPNs, and CNAs, to ensure high-quality resident care.
  • Assess, plan, implement, and evaluate individualized resident care plans in accordance with physician orders and facility policies.
  • Provide direct nursing care, administer medications, treatments, and IV therapy as required.
  • Monitor residents' conditions, identify changes in health status, and communicate promptly with physicians, families, and the interdisciplinary team.
  • Ensure compliance with federal, state, and facility regulations, including CMS, OSHA, and infection prevention standards.
  • Serve as the clinical resource and leader during assigned shifts, providing guidance, coaching, and support to nursing staff.
  • Oversee accurate and timely documentation in the electronic medical record (EMR), ensuring compliance with regulatory requirements.

  • Active RN License for NY state.
  • Minimum of 1 year of Supervisory experience Long Term Care (LTC), Skilled Nursing Facility (SNF), or Rehabilitation experience preferred.
  • Current BLS Certification (if required by the facility).
  • Strong communication, teamwork, and resident care skills.
  • Ability to work flexible days, including weekends and holidays as needed.
