RN (Telemetry)

Summary:

• Days: 7AM - 7:30PM, Every Other Weekend Required

Responsibilities:

• Deliver patient care by incorporating the tenets of the professional practice model of Relationship Centered Care.
• Assess and evaluate patient care needs and apply critical thinking skills in patient care management.
• Integrate relevant assessment and intervention skills in the delivery of nursing care.
• Collaborate with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
• Prioritize all aspects of patient care including teaching, rounding, coaching, and planning “after hospital care.”
• Educate patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease.
• Communicate effectively and professionally with patients, family, and all members of the Interdisciplinary Patient Care Team.
• Role model the tenets of Relationship Centered Care through facilitation of various patient care actions.
• Establish and maintain a therapeutic relationship with the patient and family.
• Use evidence-based practices to increase understanding of patients' perceptions of care.
• Practice safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement.
• Collaborate with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities, and nursing research studies.
• Perform charge nurse duties as assigned.
• Coordinate with the Clinical Coordinator/Clinical Nurse Manager to ensure supplies are appropriately utilized.
• Facilitate patient throughput on shift basis.

Requirements:

• 2 years of Telemetry/Med Surg experience
• BSN required
• NYS RN License
• BLS, ACLS, Tele, Ventilator certifications
• Minimum 2 years of RN experience
• Average unit census: 22
• 24 beds in unit
• Staffing guidelines: 1:5
• 3x12 schedule with weekends required

Preferred Skills:

• EPIC experience highly preferred
