Registered Professional Nurse providing safe, competent quality care based on nursing theory and research.
Incorporates the tenets of the professional practice model of Relationship Centered Care.
Applies the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards.
Responsibilities:
Assess and evaluate patient care needs and apply critical thinking skills in patient care management.
Integrate relevant assessment and intervention skills in the delivery of nursing care.
Collaborate with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
Prioritize all aspects of patient care including teaching, rounding, coaching, and planning “after hospital care” and delegating to others as appropriate.
Educate patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease.
Communicate effectively and professionally with patients, family, and all members of the Interdisciplinary Patient Care Team.
Manage assignments within the Care Delivery Model of Modified Primary Nursing and demonstrate allocation of material resources effectively.
Role model the tenets of Relationship Centered Care through various patient care actions.
Establish and maintain a therapeutic relationship with the patient and family.
Use evidence-based practices to increase understanding of patients' perceptions of care.
Practice safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement.
Demonstrate clinical skills and knowledge crucial to quality and safety in the patient population served.
Implement process improvement strategies based on institutional, evidence-based procedures.
Collaborate with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities, and nursing research studies.
Utilize institutional information technology by documenting nursing practice to support quality and performance improvement initiatives.
Requirements:
NYS RN License
BLS, ACLS, Telemetry, Ventilator experience
Minimum 2 years RN experience
Minimum 2 years Telemetry and Med Surg experience
1 year travel experience for Travel positions
EPIC experience highly preferred
Preferred Skills:
Bachelor of Science with a major in nursing preferred
Associate degree in nursing considered with relevant experience and/or active matriculation in a BSN Program
