As a Homecare Registered Nurse (RN), you will:
- Provide skilled nursing care to patients in their homes
- Complete comprehensive patient assessments and ongoing evaluations
- Administer medications, treatments, and therapies as ordered by physicians
- Monitor patient conditions and communicate changes to the interdisciplinary care team
- Develop, implement, and update individualized plans of care
- Educate patients and family members on disease management, medications, and treatment plans
- Perform wound care, tracheostomy care, enteral feeding, and other skilled nursing procedures as needed
- Maintain accurate and timely clinical documentation
- Collaborate with physicians, caregivers, and other healthcare professionals to ensure optimal patient outcomes
- Promote patient safety, independence, and quality of life
- Current Pennsylvania Registered Nurse (RN) license in good standing
- Current BLS/CPR Certification
- Minimum one year of nursing experience preferred
- Previous homecare, hospice, long-term care, rehabilitation, pediatric, medical-surgical, ICU, or acute care experience is a plus
- Strong assessment, communication, and critical-thinking skills
- Ability to work independently in a home setting
- Excellent time-management and organizational skills
- Reliable transportation and dependable attendance
