PeaceHealth is seeking an experienced RN Transfer Coordinator to support patient transfers, direct admissions, and provider consultations through our 24/7 Access Center. You'll serve as the vital connection between referring providers, hospitals, and PeaceHealth care teams, helping ensure patients receive timely access to the care they need.

What You'll Do

• Coordinate transfers, direct admissions, and consultations.
• Serve as a liaison for referring physicians and healthcare facilities.
• Facilitate communication between providers and PeaceHealth specialists.
• Support STEMI, Stroke, Trauma, Maternal/Neonatal, and other transfer protocols.
• Collaborate with Bed Management and Staffing Services.
• Perform clinical reviews and determine appropriate admission status using InterQual criteria.
• Ensure compliance with EMTALA, regulatory standards, and organizational policies.

Qualifications

• Current RN license required.
• BLS required upon hire; ACLS preferred.
• Minimum 3 years of acute care nursing experience.
• At least 1 year of recent ED, Triage, Critical Care, or Charge RN experience.
• BSN preferred.
• Transfer Center, Call Center, Charge RN, or Lead RN experience preferred.
