Job Overview
Fairview is seeking a Registered Nurse (RN) Transplant Coordinator to join our Post-Lung Transplant team, based at the Phillips-Wangensteen Building on the University of Minnesota East Bank Campus. This is a 0.9 FTE position (72 hours per pay period) and is benefit eligible.
The anticipated schedule is Monday through Friday, approximately 8:30 a.m. to 5:00 p.m., with one scheduled day off each pay period. In this role, you will provide specialized care coordination and support for post-transplant lung patients, collaborating closely with a multidisciplinary team to ensure optimal patient outcomes throughout the post-transplant journey.
The Transplant Coordinator is responsible for coordination and preparation of patients for organ transplantation and the long-term management of patients after transplant through collaboration with various members of the interdisciplinary team.
