Registered Nurse - Visiting Nurse & Palliative Care Programs

RCMS is seeking a compassionate, skilled Registered Nurse to support our Visiting Nurse and Palliative Care Programs. This role offers the opportunity to provide high-quality nursing care and support to patients and families throughout our coastal region while helping individuals navigate complex medical needs with dignity, comfort, and respect.

Compensation

  • Hourly Rate: $40 - $56, depending on experience, qualifications, and internal equity
  • Comprehensive benefits package including medical, dental, vision, retirement, and mileage reimbursement

Schedule

  • 2-3 days per week: Monday, Tuesday, Wednesday
  • 8-hour shifts
  • One weekend call shift per month

What You'll Do

  • Provide skilled nursing care, assessments, medication management, dressing changes, laboratory draws, and patient education as appropriate
  • Support patients who are homebound, transitioning from the hospital, or facing chronic, life-threatening, or end-of-life illnesses
  • Collaborate with primary care providers, specialists, patients, and families to develop and implement individualized care plans
  • Teach patients and caregivers how to manage acute medical needs and navigate available support services
  • Coordinate care and facilitate access to in-home assistance and community resources
  • Promote patient dignity, comfort, quality of life, and informed participation in healthcare decisions
  • Monitor patient status and communicate changes in condition to providers and care teams
  • Maintain accurate documentation in compliance with regulatory and agency standards
  • Travel throughout the RCMS service area to provide patient care and support

About Our Visiting Nurse & Palliative Care Programs

Coping with a life-threatening or chronic illness can be overwhelming, and many people don't know where to turn for help. RCMS's Visiting Nurse and Palliative Care Programs provide patients and families with the skilled care, support, and guidance they need during challenging times.

Visiting Nurse Services provide care to patients who have a skilled nursing need and are either homebound or transitioning from the hospital. Services may include medication management, dressing changes, laboratory draws, patient education, and other short-term clinical support.

Palliative Care provides specialized care for patients who are living with serious, chronic, life-limiting, or end-of-life illnesses. Our goal is to help patients maintain dignity and achieve the highest possible quality of life while encouraging patients and their families to be active participants in healthcare decisions.

These programs work closely with patients' primary care providers, specialists, and families to teach the management of acute medical needs, coordinate care, and facilitate access to in-home support services and community resources.

Requirements

  • Current California RN license
  • BLS
  • Valid driver's license and reliable vehicle
  • Strong assessment and documentation skills
