##### Description The Wound Care and Lymphedema Registered Nurse provides specialized care for patients with acute and chronic wounds and lymphedema. This role assesses patient needs, develops individualized care plans, delivers evidence-based treatments, and collaborates with the care team to promote optimal outcomes while providing patient education and ensuring high-quality, compassionate care. ##### Professional Duties - Provides direct nursing care, including wound care and lymphedema management, under the direction of a Physician/Practitioner and in accordance with agency policies, evidence-based practices, and the established plan of care. - Coordinates patient care services and collaborates with physicians, advanced practice providers, case managers, and interdisciplinary team members to develop and implement individualized treatment plans. - Performs initial and ongoing nursing assessments to evaluate wound status, skin integrity, edema and lymphedema severity, functional limitations, patient response to treatment, and other relevant clinical conditions. - Develops, implements, and revises the plan of care in collaboration with the Physician/Practitioner and interdisciplinary team. Establishes measurable goals and integrating wound management, lymphedema treatment, prevention strategies, patient education, and supportive care interventions. - Conducts comprehensive wound assessments and delivers appropriate wound care interventions, including dressing selection and application, wound measurement, clinical photography (as applicable), assistance with debridement within scope of practice, infection monitoring, and ongoing evaluation of healing progress. - Evaluates and manages patients with lymphedema, including edema assessment, application of compression therapy, education on compression garments, skin care management, risk-reduction strategies, and implementation of lymphedema treatment protocols within scope of practice and competency. - Assesses the patients home environment to identify equipment, supplies, compression devices, and durable medical equipment (DME) needed to support wound healing, edema control, safety, and functional independence. - Consults with and educates patients, caregivers, and family members regarding wound care, lymphedema management, underlying disease processes, treatment plans, infection prevention, skin care, safety measures, and discharge planning. - Provides patient-centered education using teaching strategies and materials tailored to the patients and caregivers learning needs, health literacy level, and individualized treatment goals. - Maintains accurate, complete, and timely clinical documentation, including assessments, interventions, treatment outcomes, patient education, and care coordination activities in accordance with organizational standards and regulatory requirements. - Communicates changes in patient condition, treatment progress, and care needs to the Physician/Practitioner and appropriate members of the healthcare team in a timely manner. - Participates in quality improvement initiatives, performance improvement activities, competency validation, and ongoing continuing education and in-service training. - Maintains current nursing licensure and all required certifications, competencies, and continuing education relevant to wound care and lymphedema management. - Perform other duties as assigned. ##### Qualifications - HS Diploma or equivalent required. Current California Registered Nurse (RN) License. Graduate of an accredited school of nursing. - 1 year of recent clinical nursing experience required. Experience in wound care, chronic wound management, home health, outpatient specialty care, or related clinical setting preferred. - Certified Lymphedema Therapist (CLT) certification preferred or must complete an approved lymphedema training program within a 30 days of hire - Ability to utilize Microsoft Office applications (Word, Excel, Outlook, Power Point) and other computer software/applications. - Ability to effectively communicate with individuals within all levels of the organization, patients, patient family members, providers, vendors and others. - Excellent customer service and telephone etiquette skills. - Ability to demonstrate compassion, professionalism, and patient-centered care in all interactions. - Current BLS certification required within 60 days of hire. ##### Physical Demands Type Description Sitting Approximately 60% of day.
