The RN is responsible for assessing wounds and skin integrity, developing and implementing care plans, monitoring healing progress, educating patients and caregivers, and collaborating with the healthcare team to achieve optimal patient outcomes.ResponsibilitiesPerform comprehensive wound, skin, ostomy, and continence assessmentsEvaluate patient skin condition and wound care needs upon admission and on an ongoing basisDevelop, implement, and update individualized wound care plans based on clinical findings and physician ordersEstablish measurable goals and recommend appropriate nursing interventions to promote healing and prevent complicationsPerform wound care treatments and dressing changes according to physician orders and evidence-based practicesMonitor wound healing progress and identify changes requiring interventionEducate patients, family members, and caregivers on wound management, skin integrity, infection prevention, and treatment plansCollaborate with physicians and other healthcare professionals regarding wound care recommendations and patient progressCommunicate significant changes in patient condition to the Director of Nursing and other appropriate care team membersMaintain accurate, detailed, and timely clinical documentationAdhere to agency policies, infection control standards, and nursing practice regulationsTreat patients, families, caregivers, and team members with professionalism, courtesy, and respectQualificationsCurrent, unrestricted Texas Registered Nurse (RN) licenseCurrent BLS/CPR Certification (in-person certification required)Negative TB skin test or chest X-ray within the past yearMinimum one (1) year of documented nursing experience in a home health agency, primary care clinic, wound care clinic, hospital, or healthcare facility preferredStrong wound assessment and treatment skillsKnowledge of evidence-based wound care practices and skin integrity managementCurrent and valid Texas Driver's LicenseCurrent automobile insuranceReliable transportationStrong communication, assessment, and documentation skillsPhysical RequirementsAbility to stand, walk, bend, squat, kneel, reach, twist, push, and pull throughout the workdayAbility to safely assist patients as needed during care visitsAbility to independently lift up to 50 poundsJoin Our TeamIf you're a compassionate and clinically skilled Registered Nurse who is passionate about wound care and helping patients achieve better outcomes at home, we'd love to hear from you.
