RN Wound Care Manager
This position is for a Registered Nurse Wound Care Manager for the Lawton Veterans Home.

Salary: $43.00 an hour
Purpose
The Wound Care Registered Nurse (RN) Lead is responsible for coordinating and overseeing the facility's wound prevention and wound management program. Under the direction of the Director of Nursing and in collaboration with the attending provider and interdisciplinary team, the Wound Care RN Lead provides assessment, treatment, monitoring, documentation, education, and clinical oversight for residents with actual or potential skin integrity concerns. The position promotes evidence-based wound prevention and treatment practices, supports regulatory compliance, and works to reduce facility-acquired pressure injuries while optimizing resident outcomes.

Knowledge and Skills
Knowledge of professional nursing principles, evidence-based wound prevention and management, skin integrity assessment, infection prevention, nutrition related to wound healing, and electronic health record documentation. Ability to perform comprehensive clinical assessments, exercise sound clinical judgment, prioritize resident care needs, communicate effectively with residents, families, providers, and the interdisciplinary team, provide staff education, analyze clinical trends, and promote quality improvement, compliance, and resident-centered care. Demonstrates leadership, professionalism, flexibility, and responsiveness to operational and resident care needs.
Other: Other duties within the Registered Nurse (RN) scope of practice as assigned.
Qualifications, Education and Experience
Minimum Qualifications
Possession of a valid, permanent Oklahoma license as a Registered Nurse in good standing with the Oklahoma Board of Nursing.
Applicants must meet one of the following experience requirements:
Experience in long-term care, skilled nursing, rehabilitation, or wound care is preferred. Demonstrated competency in comprehensive wound assessment, pressure injury prevention, infection prevention and control, evidence-based wound management, and interdisciplinary care coordination is required. Strong organizational, communication, documentation and clinical decision-making skills are essential. Candidates must possess sufficient proficiency in the English language to perform the essential functions of the position.
Target Qualifications:
Certification in wound management through the American Board of Wound Management (ABWM), such as the Certified Wound Specialist (CWS) credential, is preferred. Candidates who are not certified at the time of hire are encouraged to obtain ABWM certification, or another nationally recognized wound care certification approved by the Oklahoma Department of Veterans Affairs, within one (1) year of appointment.
Performance Measures
Core Performance Areas
Performs timely, comprehensive wound and skin assessments. Develops, implements, and evaluates evidence-based treatment plans while identifying, escalating, and intervening appropriately for new, worsening, or non-healing wounds.
Promotes evidence-based pressure injury prevention practices through routine skin surveillance, preventive interventions, interdisciplinary collaboration, and resident-centered care.
Ensures wound documentation, measurements, photography (when applicable), physician notifications, and treatment records are accurate, complete, and completed within required timeframes. Maintains survey readiness standards.
Provides clinical oversight, coaching, and education to licensed nurses and direct care staff regarding wound prevention, treatment techniques, documentation, and evidence-based skin integrity practices. Serves as the facility wound care resource.
Monitors wound care outcomes, conducts routine wound rounds and audits, identifies trends, and collaborates with the interdisciplinary team to improve clinical outcomes and reduce avoidable facility-acquired pressure injuries.
Annual Performance Goals
Ensure at least 90 percent of new or worsening wounds are assessed the same day and documented within 24 hours. Complete required weekly wound assessments and maintain at least 95 percent compliance on wound documentation audits.
Achieve and sustain at least 90 percent compliance on skin integrity and pressure injury prevention audits for three consecutive months while reducing avoidable facility-acquired pressure injuries through proactive prevention strategies.
Provide ongoing wound care education, competency support, and real-time coaching to nursing staff. Demonstrate measurable improvement in staff knowledge, wound prevention practices, treatment techniques, and documentation compliance.
Conduct routine wound rounds, communicate findings and recommendations with providers and the interdisciplinary team, monitor wound outcomes and trends, and actively participate in Quality Assurance and Performance Improvement (QAPI) activities to improve resident outcomes.
Demonstrate consistent accountability, professionalism, and flexibility by maintaining survey readiness, completing assigned responsibilities within established timeframes, supporting clinical operations as needed, and serving as the facility's clinical resource for wound prevention and management.
