    Registered Nurse (RN) Case Manager – Hospice
    Canonsburg, PA & Surrounding Area

    We are seeking a compassionate and dedicatedRegistered Nurse (RN) Case Managerto join our hospice care team. In this role, you will play a vital part in coordinating care and supporting patients and their families through every stage of the hospice journey.

    You’ll work alongside a collaborative and supportive team, delivering high-quality, patient-centered care while helping ensure comfort, dignity, and continuity of care.


    Key Responsibilities

    • Manage and coordinate comprehensive hospice care for assigned patients

    • Collaborate with interdisciplinary healthcare teams to develop and implement care plans

    • Provide clinical leadership, guidance, and support to nursing staff

    • Monitor patient progress and adjust care plans as needed

    • Ensure accurate and timely documentation in compliance with care standards


    Schedule & Work Expectations

    • Monday – Friday, 8:00 AM – 5:00 PM

    • Productivity: 6 units per day / 30 units per week

    • On-call rotation:

      • 1 weekend per month

      • 1 evening per week


    Territory

    • Primary coverage inCanonsburg, PA

    • Travel within approximately a15–25 mile radiusof the community

    • Local travel required across multiple counties
