The mission of the Wisconsin Association of Free & Charitable Clinics (WAFCC) is to support, strengthen, and advocate for the uniqueness of Wisconsin''s free and charitable clinics, the patients they serve, and the communities with whom they partner. The location for this project is Rock River Community Clinic (RRCC), a free and charitable clinic dedicated to improving access to health care and care coordination services for low-income and underserved individuals and families in its service area. The clinic works closely with community providers, hospitals, health systems, and community partners to ensure patients receive timely and appropriate care. Member Duties : The AmeriCorps Member at RRCC will strengthen access to healthcare and supportive services for low-income and underserved individuals in rural communities through care coordination, preventive health outreach, and community engagement. The member will work with Rock River Community Clinic providers, patient advocates, and community partners to coordinate referrals, track services and results, and support continuity of care. Responsibilities include prenatal and postpartum follow-up for mothers and infants, connecting patients to resources such as food, clothing, and transportation, and conducting outreach to individuals overdue for preventive screenings. The member will also help share patient success stories and clinic impact initiatives to increase community awareness and support for rural healthcare services. Through these efforts, the member will improve healthcare access, care continuity, and health outcomes while building clinic capacity.
