Position Overview:

In this role, you will play a pivotal part in sustaining and expanding El Centro Family Health School Base Health Center (ECFH-SBHC) programs. The Program Coordinator will play an integral role in the oversight of the day-to-day programmatic operations of the SBHC Program and provide administrative support to the Health Programs Director. You will bring tasks that need additional support to the Director and work together to resolve issues as they arise.

 

What You Will Do:

  • Program & Clinic Support: Organize and support Clinic Coordinators (CC) and staff at SBHC sites and local schools on meeting contract deliverables, acting as a direct point of contact for daily needs.
  • Schedule Optimization: Support Clinic Coordinators in ensuring provider schedules are completely filled at all ECFH-SBHC sites.
  • Data & Operations: Handle data entry, data reporting, and advanced data analysis to assist with the operational plan for SBHC. Incorporate statistical criteria for managing patient outcomes and administrative controls.
  • Quality Improvement & Integrated Care: Support the SBHC quality improvement (QI) team in coordinating QI implementation projects and provide administrative support for integrated, team-based care implementation at each site.
  • Outreach & Events: Attend, help coordinate, and market school/community events, including school registration events, back-to-school nights, immunization clinics, and marketing campaigns to schools and families.
  • Meeting Coordination: Coordinate and attend internal and external meetings, including collaborating with other participating agencies.
  • Patient & Provider Care: Assure targeted program patients receive coordinated clinical care with tracking and referral systems. Provide technical assistance to individual providers to clarify project procedures.
  • Compliance & Training: Perform duties consistent with Patient Centered Medical Home (PCMH) and Meaningful Use (MU) Team guidelines, utilizing certified EHR systems. Complete all required trainings and adhere to professional ethics and customer service standards.

 

Minimum Qualifications:

  • Education: High School Diploma or Equivalent.
  • Experience: Six months to one year of experience working in a healthcare or school setting. Experience working with spreadsheets and databases.
  • Certifications:
    • Ability to pursue state-level certification for a Community Health Worker (CCHW) or Health Educator (CHES) within 1 to 2 years of employment.
    • Ability to obtain Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) within 90 days of hire.
  • Intermediate proficiency with video conferencing (Zoom, Skype, etc.) and comfort being on camera/radio. Proficient in MS Office.
  • Excellent oral, written, and interpersonal communication skills.
  • Demonstrated ability to implement prevention and disease management programs, and deliver culturally appropriate, psycho-educational interventions.
  • Other Requirements: Tuberculin Skin Test (TST) clearance, Hep B Vaccine or signed Declination, a valid driver's license, and clearance for unrestricted NM automobile insurance to drive to outlying locations. Willingness to travel to other SBHC locations.

 

Preferred Qualifications:

  • Education: Bachelor's Degree.
  • Experience: Two years of experience working in a fast-paced healthcare setting directly with patients and healthcare professionals. Experience working with grants and meeting grant deliverables.
  • Language: Bilingual (English / Spanish).
  • Certifications: Pre-existing Community Health Worker certification (CCHW) and/or Certified Health Education Specialist (CHES). Pre-existing CPR or BLS certification.

 

Physical Requirements:

  • Balancing, bending, lifting, carrying, climbing, and pulling up to 20 lbs.
  • Sitting and standing for extended periods of time (four hours or more).
  • Working a flexible schedule, including evenings, weekends, or as needed.
  • Hand & finger dexterity/feeling, hearing, vision, talking, and walking.
  • Stooping, crouching, crawling, and kneeling.
