    At Bristol Hospice, we are dedicated to providing compassionate, patient-centered end-of-life care. We are seeking an organized, detail-oriented Scheduling Coordinator to support our clinical team by ensuring patients receive timely, coordinated care services.

    Coverage Area: Madison Office

    Work Schedule: Monday - Friday 8am - 5pm

    An Average Day:

    (Includes, but not limited to)

    • Review clinician availability, patient visit frequencies, and outstanding scheduling needs for the day.
    • Schedule hospice nurse and hospice aide visits in the electronic medical record (EMR) based on patient care plans and staff availability.
    • Coordinate same-day schedule changes and respond quickly to urgent patient care needs.
    • Communicate visit updates and scheduling changes with clinicians, patients, families, and caregivers.
    • Monitor visit frequencies to help ensure compliance with hospice care plans and regulatory requirements.
    • Track missed visits and provided reports to supervisors and case managers for follow-up.
    • Collaborate with the clinical team to support urgent medication deliveries, durable medical equipment (DME) requests, and other patient care needs.
    • Update scheduling records and maintain accurate documentation within the EMR system.
    • Problem-solve scheduling conflicts, staffing gaps, and last-minute visit requests while maximizing efficient use of clinical resources.
    • Provide exceptional customer service and support to internal teams, patients, and families throughout the day.
    • Maintain confidentiality and adherence to Bristol Hospice policies and regulatory standards.
    • Other duties as assigned

    Requirements:

    • High school diploma or equivalent required.
    • Minimum of 2 years of scheduling experience in a hospice, home health, or healthcare environment.
    • Experience using healthcare software and electronic medical record (EMR) systems.
    • Ability to work effectively in a professional office environment.
    • Valid driver's license and insured vehicle, as required by organizational and state guidelines.
