Care Team Coordinator / Scheduler

Location: Rhode Island & Connecticut

Schedule: Full-Time, Exempt

Compensation: $55,000 - $60,000 annually + Performance-Based Bonus Opportunities


Our client, a growing home care organization serving Rhode Island and Connecticut, is seeking a highly organized, relationship-driven Care Team Coordinator / Scheduler to help ensure exceptional care for clients while creating an outstanding experience for caregivers.


This is much more than a scheduling position.


This role sits at the center of the organization and serves as a critical link between clients, caregivers, recruiters, clinical staff, and leadership. Every schedule built, every challenge solved, and every caregiver matched directly impacts the quality of care delivered to seniors and families.

Key Responsibilities

Care Coordination & Scheduling

  • Match caregivers with clients based on skills, personality, availability, and continuity of care
  • Build and maintain efficient caregiver schedules
  • Coordinate new client starts and service transitions
  • Resolve call-offs and last-minute scheduling challenges quickly and effectively
  • Maintain strong communication with caregivers, clients, and families
  • Ensure continuity of care whenever possible

Relationship Management

  • Build trusted relationships with caregivers and clients
  • Serve as a primary point of contact for scheduling-related needs
  • Resolve concerns professionally, compassionately, and efficiently
  • Deliver an exceptional customer service experience
  • Promote caregiver engagement and client satisfaction

Operational Excellence

  • Monitor staffing coverage and identify service gaps
  • Improve caregiver utilization and schedule efficiency
  • Assist with reducing overtime and staffing challenges
  • Support recruiting, onboarding, and retention efforts
  • Maintain accurate schedules and documentation
  • Ensure compliance with company policies and standards

Experience with scheduling software such as WellSky, AxisCare, HHAeXchange, AlayaCare, or similar platforms is a plus, but training will be provided for the right candidate.
