Purpose
The HealthCare Coordinator II (HCC II) provides advanced operational and administrative support for School-Based Health Centers (SBHC) and ensures smooth, high-quality clinic operations across three (3) OR more assigned school sites. This position functions with a high degree of independence, offering enhanced operational expertise to eight (8) or more medical, dental, and mental health providers and healthcare educator(s). The HCC II serves as the primary point of contact for external stakeholders, including school district administrators, community organizations, and collaborating agencies. This role leads and participates in community outreach efforts and school- or district-sponsored events to increase student and family engagement with SBHC services. The HCC II may also provide remote coverage support for other SBHC sites as needed and contributes to the onboarding and training of new Health Care Coordinators, supporting consistent workflows and operational excellence across the program. The HCC II upholds Neighborcare Health's mission, goals, service commitments, and equity-focused care principles.
