Coverage Territory: North Shore

Danvers, Georgetown, Groveland, Marblehead, Merrimack, Newbury, Newburyport, Peabody, Rowley, Salem
Monday-Friday, 40 hours/week
Home based-schedule field visits

Under the administrative direction of the Clinical Manager, the Nurse Case Manager (NCM) for Senior Care Options (SCO) plays a vital role in coordinating compassionate, high-quality care for a panel of frail older adults. Working as part of an interdisciplinary healthcare team, the NCM partners closely with physicians, care team members, patients, and caregivers to identify risk factors, complete assessments, and develop individualized care plans that support the best possible outcomes.
Requires minimum 1 year of Home Care RN Case Management experience and/or 1 year of Care Management and familiarity with MDS-HC data set completion.
